What formula is used for creatinine clearance?

The Cockcroft–Gault equation: CrCl = ((140 − age) × weight in kg) ÷ (72 × serum creatinine) × 0.85 if female. It is commonly used for drug dosing.

Should I use actual or ideal body weight?

Cockcroft–Gault traditionally uses actual body weight, but ideal or adjusted body weight is often preferred in obesity. Follow your local protocol.

Is creatinine clearance the same as eGFR?

No. Creatinine clearance (Cockcroft–Gault) estimates clearance for dosing; eGFR (CKD-EPI) estimates GFR normalised to body surface area for CKD staging.
